Inleiding


Al een hele tijd circuleert er een hardnekkige e-mail over leptospirose en drankblikjes. Volgens deze e-mail kan men leptospirose oplopen door het drinken uit blikjes die aan de buitenkant besmet zouden zijn met rattenurine. Bijzonder aan deze e-mail is dat situatie zich zou hebben voorgedaan in België. Aangeraden wordt om de bovenkant van alle blikjes te reinigen met water en detergent alvorens eruit te drinken. Deze e-mail heeft er zelfs voor gezorgd dat de Coca-Cola Company een bericht geplaatst heeft op haar Belgische website onder de rubriek “Frequently Asked Questions” (FAQ). Anno oktober 2004 heeft J-M Bouquiaux (Premier Attaché van het Ministère de la Région Wallonne - Direction générale de l'Agriculture Division de la Politique Agricole - Direction de l'Analyse Economique Agricole, Namur) op de webstek http://www.gezondheid.be om een antwoord gevraagd of het bericht klopte over het potentiële besmettingsrisico van leptospirose via blikjes. René Snacken van het Wetenschappelijk Instituut Volksgezondheid (WIV) formuleerde een kort antwoord: deze transmissieweg is mogelijk, maar het ziekteverloop leek weinig waarschijnlijk. Allerlei andere instanties waaronder de Amerikaanse Centers for Disease Controle and Prevention (CDC) en Leptospirosis Information Center (LIC), hebben ook op dit bericht en gelijkaardige berichten gereageerd.

Achtergrond

Het internet en e-mail verkeer spelen steeds vaker een rol op het vlak van informatie. Dit is ook het geval wat betreft infectieziektes. Het Leptospirosis Information Center (LIC) heeft het bericht beoordeeld als een “fake” bericht. Hoewel het wereldwijde web vele mogelijkheden biedt, kan het voor onduidelijkheid en verwarring zorgen. “Broodje aap verhalen”, “urban legends” (stadslegendes), “hoax” en kettingmails circuleren en blijven circuleren.

Cola crisis
In 1999 werd België al eens opgeschrikt door een “coca-cola crisis”, kort na een belangrijke voedselcrisis, met name de “dioxine crisis” (ref). Na grondig onderzoek werd massahysterie (“mass psychological illness”) als oorzaak aangegeven door verschillende auteurs. Waarschijnlijk heeft de Coca-Cola Company geleerd uit deze crisis, en heeft zij daarom een bericht geplaatst op haar website over de blikjes.

Stadslegendes
Iedereen kent wel de verhalen in de trant van: “weet je wat er gebeurd is met een vriend van een vriend van mij, die ... “ Het verhaal wordt mond-op-mond verder doorverteld of zelfs lichtjes gewijzigd. Zo’n incident kan gebeurd zijn, maar is geen evidentie dat het ooit voorgevallen is. Stadslegendes (“urban legends”), ook “broodje aap verhalen” genoemd, zijn apocriefe verhalen die verteld worden als waar gebeurde verhalen, plausibel genoeg zijn om te geloven, vaak met een gruwelijk of prikkelend karakter of iemand sterk in verlegenheid brengen. Ze worden verspreid via interpersoonlijke communicatie, slechts zelden door massamedia. Omdat ze altijd maar herhaald worden door verschillende mensen op verschillende plaatsen, verandert het verhaal in de loop van de tijd zodat stadslegendes vele varianten kennen. De term “urban legend” werd in 1981 geïntroduceerd door het boek van J. H. Brunvand “The Vanishing Hitchhiker: American urban legends and their meanings”, dat opent met het verhaal van de verdwenen lifter van W. W. Norton. Stadslegendes zijn een complex en subtiel sociaal fenomeen, een soort volksverhalen die meestal vals zijn, slechts zeer zelden waar(gebeurd) zijn of gebaseerd zijn op een waargebeurd feit.
Een typische stadslegende heeft de volgende kenmerken: een volks karakter, het waarheidsgehalte is in vraag te stellen, het verhaal is zogenaamd waargebeurd, is plausibel genoeg om geloofwaardig te zijn, heeft een spontaan karakter, meerdere varianten van het verhaal circuleren, het komt van een geloofwaardige “second-hand” bron (de echtgenoot van de kapster van mijn zus, vriend van een vriend, etc.), gaat van individu naar individu in orale ofwel schriftelijke vorm (fax, fotokopij, e-mail) en is een verhaal met een waarschuwing (“cautionary tale”).
Hoax
Hoax is het Engelse woord voor poets, nep, bedrog, truc of oplichterij. Het woord is waarschijnlijk een samentrekking van “hocus pocus”. De graancirkels bijvoorbeeld worden beschouwd als een geval van “hoax”. In de computerwereld wordt de term gebruikt voor een vals bericht, een bewering die niet waar is en per e-mail wijdverspreid wordt. Typische gevallen betreffen mails met bijzonder lucratieve aanbiedingen (sommen geld), reddingacties voor zieke kinderen of waarschuwingen voor uitzonder gevaarlijke computervirussen. Vaak is de mail vergezeld van het verzoek om het bericht naar iedereen door te sturen. Een hoax leidt in veel gevallen tot onnodige opwinding en soms zelfs tot paniek. Het internet wordt belast door de vele onnodige e-mails die verzonden worden.

Methode

Het internet werd afgespeurd naar alle informatie betreffende o.a. Coca-Cola, blikjes, leptospirose, hantavirose, rattenurine, België, leptospirosis, Weil’s disease, hantavirosis, hantavirus, (soda) cans, rat, rat droppings en Belgium. Ook via PubMed werd relevante literatuur gezocht over soda drinks, cans, leptospirosis, Weil’s disease.

Resultaten

De Nederlandstalige versie van het bericht:

Artikel verschenen op "Caducée", professionele informatiebron in de medische sector.
Onlangs is een persoon in absurde omstandigheden gestorven. Hij was op een zondag per boot met vrienden vertrokken en had blikjes drank in de frigo van de boot gezet. De volgende dag werd hij opgenomen in het ziekenhuis, waar hij 's anderendaags overleed. Een autopsie heeft uitgewezen dat het ging om een snel intredende infectie met leptospiren, veroorzaakt door een blikje waaruit hij op de boot gedronken had, zonder de inhoud in een glas te gieten. Het onderzoek van de blikjes heeft bevestigd dat zij besmet waren door urine van een rat, die leptospiren bevatte. De persoon in kwestie heeft vermoedelijk het bovenste deel van het blikje niet gereinigd vooraleer er van te drinken. Het blikje was besmet door urine van een rat, die giftige en dodelijke stoffen bevat, waaronder leptospiren, die leptospirose veroorzaken. Drank in blikjes, en andere voedingswaren in gelijkaardige verpakking, worden gestockeerd in magazijnen die dikwijls wemelen van knaagdieren. Zij worden vervoerd naar de verkooppunten, zonder eerst gereinigd te worden. Telkens wanneer u een blikje koopt, reinig zorgvuldig het bovenste deel met water en een detergent, alvorens ze in de ijskast te zetten. Beter nog, koop geen blikjes. Volgens een studie van INMETRO (Spanje) zijn de deksels van drankblikjes méér besmet dan openbare toiletten. De studie wijst uit dat de hoeveelheid kiemen en bacteriën op de deksels van drankblikjes zo erg is, dat ze absoluut met water en een detergent moeten gereinigd worden. Gelieve deze boodschap aan zoveel mogelijk mensen te verspreiden. Ontvangen van de Personeelsdirectie van het kantonaal hospitaal van Genève.

We are always sceptic about can's hygienic conditions in Bangladesh (the way they are kept). When you're trying to quench your thirst with a can of Coke then you hear a family member crying "wash the top before you drink". Though many don't have the time and means to do that. But after hearing this I think they would have a second thought.

A stock clerk was sent to clean up a storeroom in Maui, Hawaii. When he got back, he was complaining that the storeroom was really filthy, and that he had noticed dried mouse or rat droppings in some areas. A couple of days later, he started to feel like he was coming down with stomach flu, complained of sore joints and headaches, and began to vomit. He went to bed and never really got up again. Within two days he was severely ill and weak. His blood sugar count was down to 66, and his face and eyeballs were yellow. He was rushed to the emergency at Pali-Momi, where they diagnosed to be suffering from massive organ failure. The doctor, in ER, transferred him to St. Francis Hospital and notifies all his relatives as he didn't had long to live. In ICU of St. Francis Hospital, a team of doctors were trying desperately to stabilize him. They said that even if by some miracle he pulled through, he would need a new liver, kidneys, pancreas and bladder. He died shortly before midnight. No one would have made the connection his job and his death, had it not been for a doctor who specially asked if the had been in a warehouse or exposed to dried rat or mouse droppings at any time. They said there is a virus (much like Hanta virus) that lives in dried rat and mouse droppings. Once dried, these droppings are like dust, and can easily be inhaled or ingested if a person does not wear protective gear of fails to wash face and hands thoroughly. An autopsy was performed on the clerk to verify the doctors' suspicions and tissue samples have been sent to the CDC in Atlanta. This is why it is extremely important to ALWAYS carefully rinse off the tops of canned sodas or foods, and to wipe off pasta packaging, cereal boxes, and so on. Almost everything you buy in a supermarket was stored in a warehouse at one time or another, and stores themselves often have rodents. Most of us remember to wash vegetables and fruits but never think of boxes and cans. The ugly truth is, even the most modern, upper-class, super store has rats and mice. And their warehouse most assuredly does! The investigation of soda cans by the Centers for Disease Control in Atlanta discovered that the tops of soda cans can be encrusted with dried rat's urine, which is so toxic it can be lethal. Canned drinks and other foodstuffs are stored in warehouses and containers that are usually infested with Rodents, and then they get transported to retail outlets without being properly cleaned. Bespreking

Op het internet werd zowel de stadslegende teruggevonden in vele talen (o.m. Spaans, Duits), alsook vele websites die het bericht afdoen als “vals”. De websites die het bericht ontkrachten, trachten daarenboven vaak de internetgebruiker correcte informatie aan te bieden. Dit gebeurt ofwel via diverse links naar andere internetbronnen, of door direct de informatie aan te geven op de webstek zelf (met bronvermelding).

Rattenurine wordt bestempeld als “toxisch en klaarblijkelijk lethaal”. Ratten kunnen dragers en verspreiders van ziekte zijn, maar misleidend wordt elke rattenurine als dodelijk omschreven.

De origine van deze stadslegende ligt waarschijnlijk bij de “outbreaks” van leptospirose in Azië en Centraal-Amerika in 1998. Daarenboven werden de Verenigde Staten van Amerika in de jaren ’90 opgeschrikt door het “Hantavirus Pulmonary Syndrome” (HPS), een vorm van hantavirose met vaak fatale afloop (tot 50%) waarbij de besmetting opgelopen wordt via muizen- of rattenurine (aërosol). De oorsprong van deze stadslegende wordt gesitueerd bij de columns van de Amerikaan (woonachtig in Thailand) Bernard Trink, verschenen in de krant Bangkok Post ("Illness Caused by Rat Urine Kills 22" (4/11/1998) en "Uncommon Rat Urine Disease Kills 8, Hospitalizes 100" (13/11/1997).
Leptospirose is potentieel dodelijk maar verloopt vaak ook onopgemerkt (subklinisch en asymptomatisch). De ziekte kan behandeld worden met antibiotica en de meeste patiënten herstellen.

Conclusie

Het internet biedt vele kansen op informatie voor bevolking, consumenten, klanten, patiënten, werknemers etc., maar houdt tevens het gevaar in van desinformatie. Vaak is er ook een commercieel doel, wat niet altijd duidelijk is in de eerste oogopslag of eenvoudig niet staat vermeld. Gebruikers moeten steeds bedacht zijn nepberichten en steeds kritisch staan ten aanzien van de aangeboden informatie. Dit gaat nog meer op voor wat betreft medische informatie. Bijkomende vaststelling is dat de online informatie snel veroudert, zeker met de evoluerende stand van de wetenschap, maar toch online blijft “voortwoekeren”.
In vele landen biedt de overheid soelaas door op officiële websites betrouwbare informatie aan te bieden aan het publiek c.q. bevolking over volksgezondheidsrisico’s. Die kunnen een tegenwicht vormen voor deze “e-mail fliers”. De informatie verstrekt door de Belgische overheden moet zeker nog verder uitgebreid en actueel gehouden te worden naar analogie met het buitenland.
Het internet vormt een constante uitdaging voor gebruikers en aanbieders van informatie (Coiera, BMJ, 1996). Een patiënt heeft bijvoorbeeld andere verwachtingen over informatie die hij op internet wil vinden dan artsen (Van Puyenbroeck & Van Royen).

Samenvatting

Dit artikel beschrijft de stadlegendes (broodje aap verhalen) en internet hoax over gezondheidsrisico’s van rattenurine op het wereldwijde web. Verscheidene verhalen over mensen die gestorven zijn tengevolge infectieziekten zoals leptospirose en hantavirose na in contact te zijn geweest met rattenurine.

Het antwoord van Coca-Cola: Dit gerucht is uit de lucht gegrepen en wordt formeel ontkend op de website van Caducée, de medische organisatie die als bron wordt vermeld in de e-mail waarin dit gerucht wordt verspreid. De productie, de opslag en de distributie vinden plaats in strikte hygiënische omstandigheden. Zo worden onze blikjes gespoeld alvorens ze worden gevuld en worden ze op de productielijn zelf verpakt. Vervolgens worden de palletten en multi-packs met blikjes opnieuw verpakt in plastic folie alvorens ze aan onze klanten worden geleverd. Het risico dat de bovenkant van de blikjes wordt besmet is bijgevolg extreem gering. Bovendien raden wij onze klanten en consumenten aan om onze producten op een droge, schone en frisse locatie op te slaan, beschut tegen het zonlicht. Als u twijfelt kunt u steeds het deksel van het blikje spoelen en/of schoonwrijven alvorens u van het blikje drinkt.

CDC – Hantavirus:

Ratten zijn geen populaire dieren en worden vaak geassocieerd met de dood en ziektes zoals de pest, hantavirose en leptospirose. Het is dan ook niet verwonderlijk dat het algemene publiek aanneemt dat ratten op zichzelf lethaal kunnen zijn. Het bovenstaande over de verhaal lijkt op een scenario op de website van de Centers for Disease Control and Prevention (CDC) “All about Hantaviruses” (National Center for Infectious Diseases) http://www.cdc.gov/ncidod/diseases/hanta/hps/noframes/aerosol.htm.
Over een individu dat een zelden gebruikte ruimte besmet met excreties van ratten opruimt.

De State of Hawaii Department of Health alsook CDC hebben het bericht bekeken en komen tot de slotsom dat de e-mail vals is. Er zijn voor zover bekend is er nog geen geval van hantavirose gerapporteerd in Hawaii. De e- mail is een “hoax”, of een “misinterpreatie” van een andere gebeurtenis.

Rattenurine is toxisch voor mensen. Een clerk in Hawaii is gestorven. De potentieel dodelijke ziekte leptospirose kan opgelopen worden door blootstelling aan urine van zieke dieren (bv. ratten).
De Centers for Disease Control and Prevention (CDC) werd geïnterpelleerd over een e-mail over een stock clerk geïnfecteerd met hantavirose. Contaminatie met opgedroogde rattenurine.
Waarschuwing dat soda cans en grocery verpakkingen (zoals cereal boxes) gecontamineerd zijn met hantavirus.

De e-mail is onwaar. Er is geen incident geweest in Hawaii. Schrik en paniek door vleugje plausibiliteit. Er is niets inherent toxisch aan urine of faeces van een gezonde rat. Excreties van ratten kunnen voor gezondheidsproblemen zorgen.
Leptospirose, een potentieel dodelijke ziekte die van dieren op mensen kan overgaan. Zwemmen in gecontamineerde meren en reservoirs. Als de rattenpopulatie stijgt in (groot)steden, neemt het potentiële contact met leptospirose toe. In extreme gevallen na 3 tot 6 dagen dodelijk.

Blikjes zijn niet de meeste verwachten infectiebron. De blikjes worden in kartonnen boxes geplaatst op de productielijn. De frisdrankproducenten proberen het product zo snel mogelijk bij de klanten te brengen. Er zijn geen grote voorraden, zodat knaagdieren weinig tijd krijgen om de verpakkingen te besmeuren. Het is altijd verstandig om de bovenkant van het blikje te reinigen, maar dan om voedselhygiënische redenen (HACCP) omdat het in andere mensen hun handen geweest is.

Er is een theoretisch risico dat mensen besmet worden door rattenurine aanwezig op blikjes, het statistisch risico is extreem klein. Leptospiren vereisen immersie in water, uitdroging doodt de bacteriën zeer snel. Het oppervlak van blikjes is niet-poreus zodat er snelle opdroging plaatsvindt. Er is in de literatuur nog geen enkel geval ondubbelzinig toegeschreven aan blikjes. En zijn ook geen studies gepubliceerd over bacteriële verontreiniging van blikjes en publieke toiletten.

Wat is leptospirose?
Leptospirose is een potentieel ernstige bacteriële infectie, die meestal opgelopen wordt in tropische gebieden.
In België worden er per jaar slechts enkele (ernstige) gevallen gemeld.
Besmette wilde en huisdieren kunnen via hun urine de leptospiren verspreiden. Leptospirose wordt opgelopen via contact met zoetwater, vochtige grond of beplanting die gecontamineerd werd door urine van besmette dieren.
Leptospirose is te behandelen met antibiotica.
Om leptospirose te voorkomen, beperk het contact met potentieel besmet zoetwater en modder, alsook potentieel besmette dieren.
Wat is de oorzaak van leptospirose?
Leptospirose wordt veroorzaakt door Leptospira interrogans, een kurkentrekkervormige bacterie (spirocheet).
Hoe wordt leptospirose opgelopen/verspreid?
Contact met urine of weefsel van besmette dieren of met water of materialen die met urine van besmette urine zijn verontreinigd, via wondjes of slijmvliezen.
Wat zijn de symptomen van leptospirose?
De symptomen van leptospirose zijn koorts, hoofdpijn, rillingen, spierpijnen, oogontsteking, braken, geelzucht, bloedarmoede en soms huiduitslag.
Hoe snel na blootstelling ontstaan de symptomen?
De incubatieperiode is meestal 10 dagen, met een spreiding van 2-30 dagen.
Hoe wordt de diagnose gesteld?
Kweek van leptospiren uit bloed (eerste week van ziekte), urine (na 10 dagen) of lumbaalvocht (na 4 tot 10 dagen); serologische onderzoek
Wie krijgt leptospirose?
Landbouwers, rioolwerkers, dierenartsen, contact met water of grond besmet door dierenurine, vooral knaagdierenurine.
Zoetwaterrecreanten (zwemmers, waterskiërs, windsurfers, zeilers, vissers, kajakkers, kanoërs, rafters, speleologen, triathleten).

Wat is de behandeling van leptospirose?
Antibiotica, met name de klasse van penicillines.
Wat zijn de mogelijke complicaties?
Nierschade, leverschade, hersenvliesontsteking en longlijden.
Hoe kan leptospirose voorkomen worden?
Vaccinatie van vee en huisdieren (hond).
Minimalisatie van het contact met zoetwater, modder tijdens recreatie en/of werk.
Douche na werk of recreatie.
Gebruik van beschermingsmiddelen (waterdichte laarzen, afdekken wonden)
Inname van wekelijks antibioticum doxycycline (chemoprofylaxie) bij kortstondige blootstelling in endemisch gebieden.
